Treatment provided 7 days per week, 24 hours per day

GENERAL INFORMATION

• An alternative to sending patients home before they are ready for a home environment

• Nursing care with an emphasis on rehabilitation and working with the patient to prepare them for daily activities and living

• A home-care setting in the hospital to assist with a smooth transition from the hospital to the home

• I.V. Antibiotics

• Skilled Nursing Care

• Physical Therapy

• Speech Therapy

• Occupational Therapy

• Respiratory Therapy

• Social Services

• Mental Health Services

• Referral and discharge planning

CRITERIA

• Must have been hospitalized in an acute care setting for at least 3 days in the past 30 days

• Must be ordered by a physician

• Patient must be medically stable

• A realistic discharge plan is in place

• Rehabilitation services intervention needs to be medically reasonable and necessary

INSURANCE COVERAGE

Medicare and Medicaid cover the service for a specific length of stay.
