GENERAL INFORMATION

An alternative to sending patients home before they are ready for a home environment

Nursing care with an emphasis on rehabilitation and working with the patient to prepare them for daily activities and living

A home-care setting in the hospital to assist with a smooth transition from the hospital to the home

CRITERIA
-
Must have been hospitalized in an acute care setting for at least 3 days in the past 30 days

Must be ordered by a physician
-
Patient must be medically stable

A realistic discharge plan is in place
-
Rehabilitation services intervention needs to be medically reasonable and necessary
INSURANCE COVERAGE
Medicare and Medicaid cover the service for a specific length of stay.
Some other insurances may cover the service, but prior authorization is necessary.
