Nurses on-call 24 hours a day, 7 days per week

GENERAL INFORMATION

  • Provides comfort and relieves symptoms of those who are terminally ill

  • The Hospice Team works with each person, their family and their Physician to determine the most beneficial treatment

  • For patients with a terminal illness who have a prognosis of six months or less

  • The Hospice team is state licensed, federally certified, and JCHAO (Joint Commission on Accreditation of Healthcare Organizations) accredited

  • Available through a Physician’s referral

  • Covered under Medicare, private pay, private insurance, Medi-Cal and Medicaid and Helping Hands program, for those who qualify

  • Covers the following areas:  South Lake Tahoe area and the Carson Valley Area (including Douglas County and Carson City)

SERVICES

  • Physician services, Medical Director

  • Registered Nurse Case Managers

  • Nursing care (with 24-hour on-call service)

  • Dietitians

  • Home Health Aide/Homemaker services

  • Medical Social Services

  • Volunteer Services

  • Medical equipment and supplies related to the terminal illness

  • Medication for symptom and pain management

  • Inpatient care for acute care & short-term respite

  • Physical, Occupational and Speech Therapy Services

  • Spiritual care with chaplain or spiritual counselor of choice

  • Bereavement follow-up for family and caregivers
