Care Management

Care Management teams help patients manage chronic or persistent medical conditions such as diabetes, lung or heart conditions, hypertension, neurological conditions, or depression by partnering with patients to develop goals, improve communication with providers, and connect patients with community resources.

Care Management requires a referral from your primary care provider or specialist.

$125 for six sessions (sliding scale available to qualified individuals)
